Midwest Case Management set ups, coordinates, and monitors services which allow people to stay in their home safely. They work directly with the person’s doctor to create a comprehensive plan of care to support them in their home. Midwest Case Management supports the frail and elderly on the Elderly Waiver, as well as those with intellectual and physical disabilities, brain injuries and health issues, including Aids/HIV. On Friday, June 10th, ladies will golf at Walton Club while men golf at Fairfield Golf & Country Club. Lunch is included and sponsors are encouraged to offer games and prizes on the course. The cost is $65 per golfer which includes entry to the Par-Tee on Thursday night. Registration forms for teams of four are due to the Chamber by Friday, June 3. Thursday, June 9th, from 5:30-9:00 pm, all golfers and the public are invited to a huge PAR-TEE on the patio of the Country Club. There will be heavy appetizers, a silent auction, raffles, and a performance by Steve and Michaela McLain. Sponsors and guests are encouraged to come and mingle with the golfers. The cost for entrance to the Par-Tee for all non-golfers is $25 and RESERVATIONS ARE REQUIRED. This is the Chamber’s biggest fundraising event of the year and we couldn’t do it without the support of our members. We are expecting a larger than ever turnout this year and there is already buzz about the exciting new changes! Meet CEO, Bryan Hunger who was most recently the Chief Provider Services Officer at Regional Medical Center in Manchester, IA. Come and mingle with the staff of JCHC and get updates on the new addition and remodel which includes connecting the JCHC Clinics the rest of the hospital with a glass corridor. They will also be adding larger rooms for the pharmacy and imaging department. Be sure to check out the tours of the 12 new fitness equipment stations on the walking trail. Enjoy seasonal appetizers, lemonade and wine in a JCHC wine glass you can take home as well as lots of other door prizes.
