SunnyBrook Living Care Center Embracing quality care with a personal touch! 24-Hour Skilled nursing Services - Specific services are provided by licensed nurses. Services may include, but are not limited to: I.V. therapy, medication therapies, wound care, catheter care and peg tube feeding. Respite Care - Temporary care is provided by SunnyBrook in the event that the primary care-giver becomes unavailable due to illness or necessity to be away for a period of time. Post Hospital/Surgical Care - A level of continued care which requires skilled nursing, but not at the acute hospital level. This care allows the client to recuperate sufficiently so that he or she may return to their home safely. Hospice and Palliative Care - Comfort care for those with a terminal illness or in the end stages of a long term medical condition. Physician/Resident/Family Participation Care Planning - A care plan is developed by all three entities to create a set of therapeutic and personal series, which supports the client's specific needs and desires for care, thereby enhancing their quality of life. Physician/Occupational and Speech Therapy Services - Medicare approved and licensed therapists will develop and carry out a program of physician ordered therapies to facilitate optimal functionality. Restorative Nursing - Services provided by licensed nursing assistants that support ambulation, range of motion and strengthening. These programs are individualized, and aid in maintaining the progress attained from previous therapies. The goal is to keep our patients functioning at the highest level possible and to maximize their dignity and quality of life. Social/Recreational Activities - Our Activity Department offers a variety of social, entertainment and activity programs daily. These activities are developed to maintain each resident's interests and to enhance his or her physical, mental and social abilities. Participation, interaction and developing relationships promotes an overall sense of wellbeing. Meal/Dining - Community dining area will accommodate therapeutic diets and restrictions. Personal likes and dislikes are kept in high regard. Weekly Church Services - Arranged by a variety of community congregations. Private and Semi-private Rooms Available Payment for Services - We accept private pay, Medicare A and B, Medicaid and Long-Term Care Insurance.
